We’re here to help you reach your health and wellness goals, and we know that takes more than appointments and referrals alone. At Hawai‘i Island Community Health Center, we use a nationally recognized model of care called the “Patient Centered Medical Home,” or “PCMH.” As a PCMH, our goal is to provide care that treats you as whole person, with an integrated team approach that delivers the care you need, when you need it. We know that each patient has unique needs —and at HICHC, you’ll have a team of caring professionals working to meet your needs and help you reach your health and wellness goals.

Your Care Team

As the patient, you are at the center of this model of care; and you have a whole team on your side ready to help with a care plan that meets your needs. This team varies depending on your needs, but will include your medical provider plus support staff like nurses, medical assistants, patient navigators, behavioral health pro- viders, or even your dentist. This team is here to support you through all of our services: Primary Care, Women’s Health, Behavioral Health, Pediatrics, Dental Care, Substance Use Treatment, Pharmacy, WIC services, and more.

Our Commitment as Your PCMH

HICHC’s pledge is to work with you, to share the decision-making in your care for your best outcomes by:Primary Medical Services:

  • Following the highest evidence-based standards of care, and providing tools, resources and education to help you improve your health;
  • Communicating with your specialists, hospitals, and other care facilities;
  • Offering whole-person care, including access to behavioral health providers;
  • Giving you opportunities for feedback and to participate in your own success;

  • Sharing resources for your health.

Your Role in a PCMH:

Because a PCMH is all about you, be sure to:

  • Be proactive with your care team to set goals for your health;
  • Bring a list of concerns and questions to discuss with your provider;
  • Bring a list of your medications;
  • Be open with your care team; share your symptoms, concerns, and any feelings of anxiety or sadness that you might have.
  • Tell us about any recent hospital visits or specialist care that you’ve had; and
  • Follow through on annual physicals and recommended screenings.
