Health Equity
Our department of Health Equity is here to help ensure that all members of our community have advocates for their health and receive the care and resources they need. We focus on addressing Social Determinants of Health (SDOH), improving the conditions in which people are born, grow, live, work, and age to help ensure health, mental health, and well-being.
Please note that not all services are available at all locations, or at all times.
What is Health Equity? As defined by the CDC, Health equity is achieved when every person has the opportunity to “attain his or her full health potential” and no one is “disadvantaged from achieving this potential because of social position or other socially determined circumstances.”
Please note that not all services are available at all locations, or at all times.
